Long Term Services and Supports - Community Partners Program:

The Community Partners program provides community-based experts designated by MassHealth who work with members to be sure that their health and wellness needs are met.  Community Partners can work with your Primary Care Provider or health care plan to address physical health, mental health and long-term care needs.

Who is eligible?

This program assists MassHealth-enrolled individuals (ages 3 to 64 years of age) who have complex medical needs, developmental or acquired disabilities, or a high need for long-term services and supports.  They may also have frequent visits to the emergency room or a recent stay in a short-term care facility.

Who works with Community Partners?

CPs work with ACOs (Accountable Care Organizations) and MCOs (Managed Care Organizations). They also work with doctors, nurses and other care providers. CPs make sure everyone has the same information and that everyone works together, putting the member at the center of care.

If a member has not been identified by MassHealth, can they still access Community Partner supports?

Yes! CP supports must be approved by an ACO or MCO. Members can be referred by a hospital, doctor, nurse or other care provider. Family Service Association can request services on behalf of eligible members. Members can also directly request CP supports from the ACO or MCO.

  • Create a plan to manage health needs and meet goals
  • Learn about and select community resources
  • Access the services members need and want
  • Navigate the health care system
  • Learn the skills needed to manage health conditions
  • Live healthy, safe lives in the community in the most independent way possible
