Home Assistance Program
for ADA
The Home Assistance Program offers daily assistance with the physical and social needs of older adults and adults with a physical disability, or a medical or mental health diagnosis. It utilizes the Group Adult Foster Care model established by MassHealth. (Despite its name, GAFC is not traditional foster care and does not involve the removal of a participant from his or her home.)
Who is eligible?
For persons at risk of losing their independent living ability, the Home Assistance Program (HAP) provides important services in the home that recognize and preserve the independence and good health of persons requiring supervision or assistance with their personal care. The member must be approved by their Primary Care Physician. Nursing and social work case management is also provided to all members enrolled in the program.
The potential member needs to be enrolled in MassHealth Standard, a Senior Care Option, or a One Care Option. There is also a Home Care Component which provides services on a Private Pay basis.
Care is provided by Certified Nursing Assistants and other professionally trained Direct Care Aides. Family Service Association can also hire a family member or friend of the member’s choice, to provide the daily personal care services, if desired.
