Adult Family Care

What is Adult Family Care? It is an Alternative to Living Alone.

With the Adult Family Care (AFC) program, qualifying adults have the opportunity to live in a family setting that provides them the best possible quality of life while remaining part of the community. This program provides a team of caring, talented professionals that provides health and care management services to its member. They carefully match each member to a home-like setting with a trained caregiver and provide on-going support and training.

Are you already caring for a family member living in your home? You may be able to receive a daily stipend for providing them with the care that they need.

Who may be eligible to become a member?

Any adult who has all of the following:

• a chronic medical or psycho/social condition, or a developmental disability and is unable to live alone
• need for assistance with at least one activity of daily living, (eg, bathing, dressing, toileting, moving between positions, ambulating, or eating.
• Enrolled in or eligible for MassHealth Standard, be enrolled in a Senior Care Option (SCO), or a One Care program.
• Personal motivation to participate in the care plan.

Who is eligible to be a Caregiver?

• A caring person who is willing to open their home to a person in need or
• A family member (other than the spouse or guardian) who may already be living in the same home, or willing to open their home their member relative. and
• A person who is willing to participate in training and work with our program to ensure the member maintains the best quality of life possible.

What Happens Now?

After both the caregiver and member agree to the match, a care plan will be developed with the help and input from both the caregiver and member. prior to move-in date. Weekly visits by the professional staff will take place during the first five (5) weeks to ensure that the medical/psycho/social care plan is implemented. Thereafter, client and caregiver will be visited by either the RN or the case manager on a monthly basis. Additional training and support for both caregiver and member is provided, and a staff member is on call 24 hours per day, 7 days a week.

What is the Cost?

Members must be on or eligible for MassHealth Standard or SCO. At present, the member contributes a daily stipend, (which may increase due to the cost of living) and the caregivers are paid a daily, tax-free stipend from MassHealth.
