Clinical Programs:
 
CLINICAL SOCIAL WORKER
 
The Clinical Social Worker provides outpatient treatment to individuals and families served by our Behavioral Health Center in our offices and / or on an outreach basis.  The Clinical Social Worker should be knowledgeable and experienced in the use of several of the following treatment modalities; individual, couple, family, child and group. IN-HOME THERAPY CLINICIAN
 
The successful candidate must be a Master’s level therapist, MA LICSW, LCSW, LMHC,  or license eligible,  providing medically necessary services to children with Severe Emotional Disturbances in the home and community setting.  Responsibilities include development of a treatment plan and, using established psychotherapeutic techniques and intensive family therapy, working with the entire family, or a subset of the family, to implement focused structural or strategic interventions and behavioral techniques.  Goals are designed to: enhance problem solving, limit-setting, risk management/safety planning, communication, build skills to strengthen the family, advance therapeutic  goals, or improve ineffective patterns of interaction; identify and utilize community resources; develop and maintain natural supports for the youth and parents/caregiver(s) in order to promote sustainability of treatment gains.  The candidate must have the skills necessary to engage and work with others from diverse backgrounds, maintain a non-judgmental attitude towards families and have the willingness and ability to work flexible hours to meet the clinical needs of the child and family.  Billable services include time spent on documentation, care coordination, collateral contact and no show/member outreach.To download a complete job description for this position, CLICK HERE.
 
 
Adult / Elder Programs:
 
PER DIEM VAN DRIVER (RAYNHAM AND FALL RIVER)

We are currently looking for dependable, conscientious individuals with a clean driving record and good communication skills and who have MA 7D licenses to work in the Raynham/Taunton and Fall River areas.  Applicants must have a proven record of vehicle safety and must provide safe and courteous transportation for our program participants, utilizing the agency's fleet of vans.

Applicants must be able to pass CPR and First Aid Certification and have the ability to physically assist program participants as they enter and exit the vehicle according to established safety rules and road regulations.  The successful applicants must also be able to successfully complete a MA State CORI check and company pre-paid pre-employment physical. SOCIAL WORKER – ADULT DAY HEALTH

The Adult Day Care Social Worker is responsible to monitor mental health status and provide case management services to program participants.  Responsibilities also include informing participants and their families/caregivers of available community services and make referrals as indications warrant.  The ideal candidate will assist, as necessary, in the delivery of other required program services and assist the Program Leader with the implementation of policies and procedures.  This is a part time, 20 hour per week position.  Education and Experience: Bachelor’s Degree is required and one year experience working with disabled/elderly adults in a community setting.  To download a complete description of this position, please CLICK HERE.

 
ACTIVITY ASSISTANT ADULT DAY HEALTH - RAYNHAM

Our Adult Day Health Program located in Raynham is currently looking for an Activity Assistant, on a per diem basis, who shall assist the Program Assistant and Activity Director with the day-to-day implementation of the goals and activities set forth by the programs daily operations.

Duties and Responsibilities:

  1. Maintain individual participant records to include program notes and daily activity logs as assigned.
  2. Assist the Program Leader through the design of new ideas to increase and ensure program growth and quality.
  3. Assist Program Leader with the implementation of policies and procedures.
  4. Assist and encourage participants with opportunities for socialization, education, group interaction and recreation.
  5. Assist in set up, implementation and breakdown of daily scheduled activities.
  6. Assist with meal procurement, service, and delivery and clean up in accordance with sanitary food preparation and USDA standards.
  7. Assist with the ADL needs of the participants as warranted.
  8. Respond to the reasonable expectations of the Agency and/or supervisors.

CAREGIVER (NEW BEDFORD, MA)
 
Our Home Assistance Program is currently looking for part-time caregivers in the New Bedford area.  Caregivers monitor the placement status in the home and provide assistance with participant's daily living needs as directed by a plan of care.  Duties may include: cleaning and general housekeeping, assisting with shopping, providing on-going supervision of the participant's health-related activities.
 
The ideal applicant must have successfully completed a Certified Nurse Assistant (CNA) and/or Home Health Aide (HHA) training program and possess a certificate of course completion and/or a valid license.  Equivalent training or experience could be substituted for certification/licensure.  For a complete description of this position, please    CLICK HERE.
 
 
CASE MANAGER - GUARDIANSHIP
 
The Family Service Association Guardianship Program offers a variety of specialized services for those needing assistance in managing their personal, medical, financial, and legal decisions, in accordance to Massachusetts General Laws.  The program is currently looking for someone to fill a full-time Case Manager position.  The Case Manager provides guardianship and conservatorship services for those determined by the court to be legally incompetent by a physician or in need of assistance with management of their personal and/or financial affairs.  Duties include but are not limited to doctor consultations; hospital admissions and discharges; maintaining treatment plans and the monitoring of personal needs.

REGISTERED NURSE - AFC (TAUNTON, MA AND FALL RIVER, MA)

We are currently looking to fill the full-time Registered Nurse position in our Adult Family Care programs.  This position is based at both our offices in Taunton and Fall River, MA.  The Adult Family Care Registered Nurse evaluates potential participants and foster families, monitors client/caregiver placement status, provides case management services, serves as liaison with assigned participants' primary care physicians, monitors participants health status, completes semi-annual health status reports and patient care plans, implements and evaluates caregiver training sessions and assists in outreach to increase community awareness of AFC Program services.

The ideal applicant must be licensed by the Board of Registration to practice in the Commonwealth of Massachusetts and in good standing. Applicant must be a Registered Nurse. Two (2) years recent experience with geriatric and disabled adult populations in a community setting is preferred. Applicant must be CPR certified. REGISTERED NURSE (FALL RIVER, MA)
 
We are currently looking to fill a per diem Registered Nurse position in our Adult Day Health program.  The RN monitors participants’ health status and is responsible to complete quarterly health reports and participant care plan. Other duties include, but are not limited to: providing case management services; supervising all other services involving health issues of the participants; and assisting the Program Leader with the implementation of policies and procedures.
 
The ideal applicant must be licensed by the Board of Registration to practice in the Commonwealth of Massachusetts and in good standing. He/she must have at least two (2) years recent experience in the direct care of the elderly or chronically disabled persons. CASE MANAGER INTERN
 
The Case Manager Intern will be directly accountable to the Client Services Coordinator of the Guardianship Program.  The intern will assist the Case Managers with management of all cases assigned.  Duties may include visiting clients, involvement with hospital admissions and discharges, placements at nursing homes and other facilities, obtaining medical certificates, preparing funeral arrangements, issues involving personal and real property and monitoring of personal needs.  The Intern will be scheduled for 10-20 hours per week.  For a complete description of this position,    CLICK HERE.
 
 
CAREGIVER (NORWOOD LOCATION)
 
The Caregiver monitors the placement status in the program participant’s home and provides assistance with daily living needs as directed by a plan of care and ensuring that personal care services are provided on a daily basis.  Duties may include: cleaning and general housekeeping, assist with shopping, provide on-going supervision of the participant’s health-related activities. CARE / CASE MANAGER (FALL RIVER)
 
Our Home Assistance Program is looking for Social Worker / Care Manager.  This person will evaluate potential participants as well as assist in outreach to increase community awareness of program services.  Duties include: evaluation of potential participants; providing case management services; developing individual multi-disciplinary care plans.  This full-time position (35 hours per week) also offers a full benefit package, including generous time off.
